Are you a Medicaid or Medicare member resident who has no means of transportation to get to and from your medical related destinations? WellTrans is here for you! WellTrans arranges transportation services for Medicaid or Medicare members who require non-emergency transportation assistance to covered healthcare services. Call WellTrans to arrange your non-emergency medical transportation service at least two (2) days in advance of the appointment date. You can schedule up to 45 days in advance.

  • Monday – Friday: 8:00 a.m. – 8:00 p.m. Eastern Time for scheduled trips

Making Transportation Arrangements:

When scheduling transportation services, please have the following information ready for the Customer Service Representative:

  • Member’s full name, phone number, address, date of birth and county of residence
  • Member’s Medicaid number as it appears on their Medicaid card
  • Specify any special needs (e.g., oxygen, escort, etc.)
  • An emergency contact’s name and phone number
  • Destination information – address, phone number, doctor’s name, type of appointment, and name of the medical facility to which you are being transported.
  • Member’s Mobility status (e.g., waking, wheelchair, etc.)

 

Gas Reimbursement Enrollment:

WellTrans provides gas reimbursement for covered members, as specified by the member’s health plan. To get started with enrollment for gas reimbursement, contact the Gas Reimbursement Team via one of the following methods:

  • Email: gas@welltransnemt.com (Include your full name, birth date, and reliable contact information.)
  • Phone: 844-772-6632 (Anthem Healthy Indiana Plan, Hoosier Healthwise, Hoosier Care Connect, or Indiana PathWays for Aging Members only)
  • Monday- Friday from 8:30 a.m.- 4:30 p.m. Eastern Time
