Making Transportation Arrangements:
When scheduling transportation services, please have the following information ready for the Customer Service Representative:
- Member’s full name, phone number, address, date of birth and county of residence
- Member’s Medicaid number as it appears on their Medicaid card
- Specify any special needs (e.g., oxygen, escort, etc.)
- An emergency contact’s name and phone number
- Destination information – address, phone number, doctor’s name, type of appointment, and name of the medical facility to which you are being transported.
- Member’s Mobility status (e.g., waking, wheelchair, etc.)
Gas Reimbursement Enrollment:
WellTrans provides gas reimbursement for covered members, as specified by the member’s health plan. To get started with enrollment for gas reimbursement, contact the Gas Reimbursement Team via one of the following methods:

- Phone: 844-772-6632 (Anthem Healthy Indiana Plan, Hoosier Healthwise, Hoosier Care Connect, or Indiana PathWays for Aging Members only)
- Monday- Friday from 8:30 a.m.- 4:30 p.m. Eastern Time
- Please ask to speak with a Gas Reimbursement Specialist
